Getting Started

   
 

Checklists

Prior to your initial surgical consultation:

  • Attend the Bariatric Surgery Information Session
  • Complete all paperwork:
    • Bariatric surgery patient questionnaire
    • Health survey
  • Obtain insurance referral, if required by your insurance provider

What you need to bring to the consultation:

  • Personal information-social security number, driver’s license
  • Insurance referral
  • Insurance card
  • Completed forms:
    • Bariatric surgery patient questionnaire
    • Health survey
  • Any questions you may have
  • Documentation of supervised weight loss programs

Highly recommended activities:

  • Complete routine well-adult care (e.g. mammograms, colonoscopy)
  • Begin walking 10 to 20 minutes per day
  • Drink six to eight glasses of water per day
  • Learn as much as you can about bariatric surgery and what lifestyle changes are required after surgery
  • Attend bariatric support group meetings
