Penn Home Infusion Therapy
Penn Home Infusion Therapy provides patients
with necessary treatment and intravenous medication
in the comfort of their own home. Our infusion
services are available in Pennsylvania, southern
New Jersey, and northern Delaware.
Our multidisciplinary team approach allows us
to provide continuity of care from hospital to
home. Penn Home Infusion Therapy is certified
by the Joint Commission on the Accreditation
of Healthcare Organizations (JCAHO).
Infusion Specialists
Penn Home Infusion Therapy has a multidisciplinary
team. All pharmacists are trained in infusion,
and someone is available 24 hours a day. Our
nurses undergo up to three months of customized
orientation. Each member of the care team is
well equipped to assess, educate and care for
infusion patients.
In addition to providing education and care,
Penn Home Infusion Therapy closely monitors the
patient's clinical status, dosage frequency,
and treatment efficiency. The expertise and extensive
experience of the care team allows them to identify,
communicate, and respond to any changes in the
patient's status related to the applied therapy.
Penn Home Infusion Therapy is dedicated to compassionate
care, and clinical and educational excellence.
In order to promote these ideals, we maintain
an active quality improvement program to continue
our tradition of exceptional care and patient
satisfaction.

As part of Penn Home Care and Hospice Services,
Penn Home Infusion Therapy is able to coordinate
with Penn Care at Home and Wissahickon Hospice
to offer a more comprehensive, efficient, and
tailored treatment program to home health care
patients, even those with multiple needs.
With additional disciplines at our disposal,
including rehabilitative therapies, home health
aides, social services support and palliative
care, we are able to offer greater scheduling
flexibility and a wider variety of care options.
Because our services are integrated, we are able
to exchange information with greater efficiency,
and therefore provide better care.
Patient Education
At Penn Home Infusion Therapy, educating patients
and caregivers in proper care techniques is
our primary concern. To this end, each patient
and caregiver receive personal education and
an instruction manual tailored to their condition
and individual needs. This personalized care
plan provides an easy-to-understand resource
for the patient, allowing the patient to regain
independence as quickly and comfortably as
possible.
Timeliness
Penn Home Infusion Therapy can expedite dependable
delivery of medication and medical supplies.
We operate our own infusion therapy pharmacy
(available 24 hours a day, 7 days a week) including
a supply warehouse and delivery staff. Medications
and supplies can be received by patients just
hours after their initial referral to Penn
Home Infusion Therapy.
Ease and Convenience
Our centralized intake and access system makes
referring a patient easy and convenient. A
single call to 866-888-8598 will put a patient's
health care provider in touch with an RN trained
in all Penn Home Care and Hospice Services
programs, and who will be able to triage a
patient for all of our services.
Our managed care representatives will take care
of all reimbursement issues, from obtaining up-front
authorizations to contacting the patient to discuss
coverage and any co-pay issues prior to start
of care.
